Our practice is firmly committed to providing you with the best possible experience when you step into our office; we want you to feel like you’re more than just a name on our schedule! We won’t rush you back when you arrive, or hurry you out the door when we’re finished; instead, we’ll treat you like the important guest you are, value your time, and make sure your teeth receive the care they need. All that said, we do ask that you bring your completed paperwork with you when you arrive, to ensure a smooth and streamlined process.

Patient Forms

In order to make things as streamlined and simple as possible during your appointment, we kindly request that you fill out your paperwork beforehand by downloading and completing the patient forms linked below (we can also text you a link to access them). You’ll need to bring these forms with you, and we’ll be able to get you entered into our system so that we can begin keeping track of your smile’s history. This also tends to reduce the amount of downtime during each visit; we know your time is important!
